It doesn't mean "horny". It means "feels good" in a sexual sense. Can also be used non-sexually, to describe an exciting feeling.

I.e., when you go down that first dip on a roller coaster, your stomach feels "siaw".

Or, if you just barely avoid having something bad happening to you, that also gives you a "siaw" feeling.

I think "see-oh" (rising tone) is probably closer than "siaw".

It took me a while to figure this term out in the bars and I didn't know how to spell it to look it up.

"jakajee mai?"

"mai chai. see-oh"

"tee nee jakajee mai?"

"see-oh"

I must be stupid, but it took me days to figure it out exactly. It was fun learning though!
