Staph infection?

A friend has asked for help. A friendly young lady gave him a not so friendly bug. Anyone have any experience with it?

 

Chula Hospital diagnosed it as a Coagulase Negative Staphylococcus that appears to be resistant to treatment.

 

He then tried Bamrungrad... they couldn't find anything and all he got was a big bill.

 

Advice???

Some quick research reveals that staphylococcus saprophyticus is coagulase-negative, resistant to Novobiocin (an antibiotic), and is the second-most common cause of urinary tract infections in females aged 17-27. That would seem to match up pretty well with the story in this case.

 

A bit more searching reveals that quinolone antibiotics are commonly used to treat s. saprophyticus infections. Cipro (ciprofloxacin) is a quinolone, and is quite commonly used in Thailand for treating common infections. Furthermore, 100 mg Cipro in a single dose will kill a lot of UTIs (not necessarily all s. saprophyticus). (One article was a study of 100 mg Cipro vs. 250 mg Cipro single-dose regimens for treating UTIs: both were very effective, and about equally so.) If Chula Hospital gave your friend a dose of Cipro, there's a good chance it killed the infection before Bumrungrad ever saw him.

 

It should go without saying that your friend should consider hauling the female in question to a doctor, immediately, to get her UTI treated.

Thanks all. He wasn't getting anywhere with the Thai hospitals and has gone back to the UK for treatment. The problem is that the bug quickly builds up a resistance to a medication. There aren't many that are effective to start with. He didn't want to waste any more time with the Thais experimenting - and possibly build up a resistance to the only drugs that would work. Haven't heard from him since he left, and I hope he's getting some success.
