I walked in, filled out some paper work had all the tests done in less than two hours. You do need to bring your passport. I can't remember the total cost but it wasn't very expensive. They e-mailed me the results of the tests that didn't have an immediate answer. They even let me buy the medicine there in the event the tests came back positive. Complete STD test was ~2000 baht or so (can't remember, but about the same or cheaper than in clinics). Anonymous testing was OK -- they do insist you give them *some* name and show *some* ID (they did not copy it or write down the real name). Courteous, professional, had the HIV and Syphillis results within 2h, Chlamydia within 7 days.
