The lady (or dude) who looks after the place keep that money for themselves. For that, they are expected to maintain the bathroom (clean -- using their own supplies -- and restock it) and keep the rest as their salary. A small price to pay to keep a clean head IMHO. And you're cheap charlies if you complain about paying 5 baht for a clean toilet without questioning if you're paying 40 or 75 baht for the beer. Go to Pattaya - sit in the bars outside Sabailand, you have 2 choice either walk over to the free toilets in Festival Center ("Big C") or to the 5 baht toilet around the corner (Soi 3).
