For the English, it is as close as you'll get being "one slice short of a loaf" or a "one can short of a six pack" etc. It derives from being one seleung short of a baht of gold and means somebody is not with it or is mentally unstable. Khun Khong, you are quite right, it refers to pu tao meaning old person (unisex). We even have a board member with that exact nick as I recall.
